Summary:This activity, Catching-up Regions 3 (CuR3), is designed as a one-year engagement with three selected regions in Poland, based on their regional needs and priorities. The activities of this initiative build on the results of the CuR Initiative delivered by the World Bank and the European Commission over the last two years across Poland. The objective of this task is to propose a service delivery model that will facilitate care for the elderly over the age of 65 years of age in Grudziadz. This effort includes care, services, investments, and new initiatives in a way that strives to prevent the institutionalization of the elderly, as long as possible. The local system of care delivery for seniors does not stand alone, and will be molded with consideration to the broader context of policy and governance arrangements for health and social care already established in Poland, together with the funding, capacity, and systems that are currently in place. All local and national stakeholders from Ministries to non-governmental organizations or local community groups can have a part in the model implementation. The model builds on the national system’s focus on the changes and adjustments that need to be made at the local level.
