Summary:In the last few years a novel scope suggest the use of Aromatase inhibitors in women with PCOS (one of the most cause of anovulatory infertility) for treating the whole spectrum of reproductive, endocrine abnormalities usually found in such subject, particularly those who are classified as Clomiphene citrate resistant patients. On the other hand the discovery of Leptin hormone,a cytokine secreted by adipocytes, (167-amino acid peptide product of ob gene), generated an extraordinary interest in the understanding of the relationship between metabolic status and the neuroendocrine system, in particular, the neuroendocrine regulation of reproduction. Moreover, leptin was thought to be involved in the regulation of implantation and has also been indirectly associated with steroid production in the ovary and in the regulation of progesterone production by granulosa cells.Brannian et al. (1999) thought that Leptin is also involved in the female fertility, especially in pubertal development. Although the exact mechanisms and nteractions with sex steroids are not yet fully established, it is clear that Leptin plays a role as an endocrine mediator in sexual development and reproduction. The concept has emerged that a normal Leptin secretion is a prerequisite for normal reproduction function, Leptin acting as a permissive factor since initiation and maintenance of normal reproductive capacity necessitate adequate metabolic conditions . Most of the researchers in the use of Aromatase Inhibitors for ovulation induction in patient with PCOS has been carried out relatively recently and some theories are still controversial, I have therefore, summarized the available experimental evidence in this introduction and explained the reasons for the development of the study reported in this book.
